Rental Agreement Form (Alpha-Stim(R) SCS) - Fax your rental agreement and Rx to 1-800-939-8907 or you can send both documents and send them to us at info@stimtherapy.com as email attachements, or you can mail them to McManus Health Systems, LLC, 76 Allen Lakeshore Dr. Santa Rosa Beach, FL 32459
• When we receive your Rx and a signed rental agreement, an Alpha-Stim® unit will be mailed to you
• Your credit card will be billed a low monthly fee for each month of rental.
• Phone support is available to assist you in learning how to use your Alpha-Stim® unit.
• “Your unit may be purchased at any time by paying the remaining balance”.
• You always have the option to terminate the rental agreement by returning the unit at any time or you may continue paying monthly rental fees until the balance is fully paid off. All rental expenses go towards the purchase price of your own device.
*Rentals require a major credit card account and a damage deposit. Monthly rental costs based on 5-month (AS-SCS) or 7-month (AS-100) rental periods.
5-Year Manufacturer’s Warranty
Each Alpha-Stim® 100 and SCS is warranted to be free from defects in materials and workmanship under normal use for a period of five years except for replacement of batteries and accessories. See Owner’s Manual for complete details.
