Step 2 – Ask Your Healthcare Practitioner if Alpha-Stim® is appropriate for you.
- Download and print this form: Alpha-Stim Order for Use
- Complete all the sections of the Order for Use form except Healthcare Provider area.
- Give the form to your Health Practitioner
- Have your Health Practitioner complete and sign the form
